Work Environment\n

This role offers the flexibility of a remote working environment, allowing you to engage with clients through virtual platforms and occasional in-person visits as necessary. We maintain a structured full-time schedule from Monday to Friday, 8:00 AM to 4:30 PM, while also providing the option for on-call support to address urgent client needs. Key Responsibilities\n

• Conduct in-depth psychosocial assessments of senior clients, evaluating their emotional, mental, and social needs.

• Collaborate with seniors and their families to create personalized care plans that align with their preferences and goals.

• Provide individual and family counseling, assisting clients in coping with life changes, transitions, and challenges specific to aging.

• Advocate fiercely for clients, helping them access essential community resources and services that support their health and well-being.

• Work closely with a variety of healthcare professionals, including nurses and caregivers, to coordinate efforts and enhance care delivery.

• Perform regular check-ins to monitor the health and safety of clients, making adjustments to care plans as needed.

• Facilitate family communication, resolving conflicts that may arise in the provision of care for senior clients.

• Educate clients and families about available community resources, support groups, and programs that foster social engagement.

• Document client interactions accurately using electronic health record systems, maintaining confidentiality and compliance with ethical standards.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in Arizona.

Compensation and Benefits\n

We offer a competitive annual salary range of USD 60,000 – 100,000, commensurate with your experience and qualifications.
