Concept Pharmacy is hiring a Full-Time Clinical Pharmacist to support our long-term care operations. This is a non-retail, clinically focused role within a Joint Commission–accredited pharmacy serving skilled nursing, assisted living, and home care facilities across Pennsylvania.

Key Responsibilities

• Verify, process, and dispense medication orders

• Supervise IV admixture preparation in sterile compounding room

• Conduct comprehensive medication regimen reviews

• Oversee medication inventory management and delivery coordination

• Serve as clinical resource for facility staff and providers

Required Qualifications

• Active, unrestricted Pennsylvania Pharmacist license (RPh or PharmD)

• Prior experience practicing as a licensed pharmacist in long-term care, institutional, or community pharmacy

• Working knowledge of IV compounding and USP standards

• Strong clinical judgment and ability to work independently in a fast-paced environment
