Key Responsibilities\n

• Conduct comprehensive mental health assessments, including psychosocial evaluations and risk assessments, to identify patients' needs in the hospice setting.

• Develop individualized treatment plans alongside patients and their families, ensuring that cultural and personal preferences are integral to the planning process.

• Implement evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT), tailored for individual, couple, family, and group settings.

• Provide immediate crisis intervention services for patients in acute psychological distress, ensuring effective coordination with healthcare teams for their care continuum.

• Maintain thorough and precise clinical documentation that includes treatment plans, progress notes, and discharge summaries, in accordance with both clinic policies and state regulations.

• Engage in psychoeducation with patients and families to equip them with the necessary tools to manage emotional and psychological challenges associated with hospice care.
\n
• Work collaboratively within a multidisciplinary team consisting of psychiatrists, psychologists, nurses, and case managers to deliver holistic care to clients.

• Conduct regular outcome assessments to evaluate the effectiveness of implemented treatment modalities, making necessary adjustments to enhance patient well-being.

• Stay current with best practices in mental health, integrating new research findings to ensure the delivery of optimal care.

• Adhere strictly to ethical guidelines while maintaining the confidentiality of patient information.

Qualifications\n

• A Bachelor's Degree in Social Work is required; a Master's Degree is preferred.

Compensation and Benefits\n

The annual salary for this position ranges between USD 70,000 and 84,000, commensurate with experience and qualifications.
