Work Environment\n

This position is designed for remote work, granting you the flexibility to manage your schedule based on your caseload and the specific needs of your patients. You will work from the comfort of your home while maintaining close communication with our hospice team and utilizing digital systems to manage patient records and treatment plans efficiently.

Key Responsibilities\n

• Maintain accurate clinical records and ensure compliance with state regulations and organizational policies.

• Collaborate with multidisciplinary teams, including physicians, nurses, and other social workers, to develop cohesive treatment plans tailored to each patient’s needs.

• Conduct thorough assessments to determine the psychosocial needs of patients and their families, and subsequently develop comprehensive treatment plans.
\n
• Deliver psychotherapy to individuals facing mental health challenges, utilizing solution-focused interventions such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT).

• Educate patients and families about mental health, substance abuse resources, and coping strategies to improve overall wellness.
\n
• Regularly review and adjust treatment plans based on ongoing assessments, ensuring that therapeutic goals are met while adhering to ethical standards.

• Stay updated on the latest research in substance abuse treatment and integrate findings into therapeutic practices.

Patient Population / Client Focus\n

In this role, you will work with individuals struggling with substance abuse as well as co-occurring mental health disorders. Our diverse patient population includes adolescents and adults from various backgrounds, each facing unique challenges that require sensitivity, understanding, and a tailored approach to care. Your interactions will not only support their recovery but also educate them and their families about the resources available for sustained success.

Qualifications\n

• A valid LCSW license in the state of California is required.

• A Master’s degree in Social Work from an accredited institution is necessary.

Preferred Experience\n

Compensation and Benefits\n
