Work Environment\n

This is a full-time, remote position, allowing for flexibility while maintaining a structured schedule. You will work standard hours from Monday to Friday, 9:00 AM to 5:00 PM, facilitating a healthy work-life balance. Key Responsibilities\n

• Act as a liaison between families and outpatient providers, ensuring effective communication and coordination of care.

• Engage with insurance companies to manage pre-certification processes and ensure effective utilization management for clients.

• Participate actively in discharge planning and create comprehensive aftercare plans tailored to individual client needs.

• Provide therapeutic meal support and guidance, modeling and fostering a healthy relationship with food for clients.
\n
• Deliver empathetic and affirming care that respects and acknowledges the unique circumstances and intersecting identities of each client.

Patient Population / Client Focus\n

You will primarily serve an adult patient population, many of whom are in hospice care, requiring not only clinical support but also emotional and psychological assistance during critical life transitions. Your role will be essential in helping clients navigate complex feelings, relationship dynamics, and the impact of chronic illness, offering individualized care grounded in respect and understanding.

Qualifications\n

• Master's degree in Social Work from an accredited institution.
