Key Responsibilities\n

As a Senior Licensed Clinical Social Worker, you will play a pivotal role in the emotional and psychological well-being of our patients. Your daily responsibilities will include:

• Conducting comprehensive assessments and diagnosing mental and emotional disorders in patients, utilizing interviews, observations, and standardized testing as appropriate.

• Designing and implementing individualized treatment plans that may incorporate therapeutic counseling, medication management, and referrals to community resources.

• Facilitating group therapy sessions, as well as individual counseling, to address varying levels of mental health conditions and foster a supportive community.

• Setting measurable and attainable patient goals, guiding discussions around treatment plans, ensuring that patients feel heard and valued throughout their journeys.

• Providing continuous support and encouragement, assisting patients in discussing their experiences and processing their emotions within a safe, empathetic environment.

• Effectively managing crisis situations and assisting patients through life transitions and challenges, employing evidence-based intervention tactics.

• Collaborating with medical professionals and other disciplines to deliver integrative care tailored specifically to each patient’s unique needs.

Patient Population / Client Focus\n

Your primary focus will be working with individuals in hospice care, who are often dealing with terminal illnesses and the accompanying emotional and psychological hardships. You will engage with patients and their families, addressing issues related to grief, loss, and end-of-life concerns, while simultaneously fostering resilience and supporting coping strategies.

Qualifications\n

• A Master’s Degree in Social Work is required.

• An active and valid LCSW license in the state of California is mandatory.

• A minimum of 2 years of clinical experience in mental health settings, particularly within hospice or palliative care, is necessary.

Compensation and Benefits\n

SageWise Wellness is pleased to offer a competitive salary ranging from USD 120,000 to 130,000 annually, based on experience and qualifications. In addition to a strong salary, our benefits package includes:
