Key Responsibilities\n

• Engage with clients on a routine basis to assess and evaluate their emotional and behavioral health, documenting insights in detailed clinical notes.

• Collaborate with other healthcare professionals, including physicians and case managers, to devise comprehensive treatment plans tailored to individual client goals and needs.

• Deliver crisis intervention services, ensuring that patients receive timely and effective support during challenging times.

• Assist in the development of discharge plans that incorporate appropriate aftercare resources and facilitate transitions into continued care, ensuring the ongoing health and wellness of clients.

• Provide family consultations to educate and involve support systems in the treatment process, thereby enhancing client outcomes.

Patient Population / Client Focus\n

The LPC will work with a diverse population of clients facing various mental health challenges, including anxiety disorders, depression, trauma, and relationship issues. Qualifications\n

• A Master's degree in Professional Counseling from an accredited institution.
