Key Responsibilities\n

• Conduct thorough, initial assessments of clients to identify their individual mental health needs and challenges.

• Develop, implement, and revise personalized treatment plans based on evidence-based practices to facilitate recovery.

• Utilize a variety of therapeutic techniques and interventions to engage clients, such as cognitive-behavioral therapy and mindfulness practices.

• Maintain accurate and timely clinical documentation in compliance with regulatory standards, ensuring confidentiality and adherence to best practices.

• Work collaboratively with a multidisciplinary team of healthcare professionals, including physicians, nurses, and support staff, to deliver holistic and integrated care.

• Educate clients and their families about mental health management strategies and available community resources to promote ongoing support and recovery.

• Stay updated on the latest advancements in mental health practices and legislation to ensure your approach remains current and empathetic.

Patient Population / Client Focus\n

Your primary focus will be on clients receiving hospice care, a population that may face complex emotional and psychological challenges as they navigate end-of-life issues. You will work closely with individuals dealing with grief, depression, anxiety, and other mental health conditions that require specialized support. Qualifications\n
