Key Responsibilities\n

• Act as a primary liaison between families and outpatient service providers, ensuring effective communication and continuity of care.

• Engage with insurance companies for the pre-authorization of services, as well as manage utilization reviews to ensure patients receive appropriate care.

• Lead discharge planning as well as aftercare services to promote ongoing support and recovery post-treatment.
\n
• Offer therapeutic meal support, guiding clients to model a healthy relationship with food and addressing any eating concerns.
\n
• Deliver empathetic, safe, and affirming care tailored to each client’s unique circumstances, considering their diverse and intersecting identities.

Patient Population / Client Focus\n

This role is primarily focused on supporting individuals undergoing detox treatment. Clients may present with various mental health challenges, substance use disorders, and co-occurring conditions. The LCSW will be instrumental in creating an affirming space where clients can explore their issues holistically, addressing both their mental and emotional health needs.

Qualifications\n

• Master's degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
