Key Responsibilities\n

• Conduct thorough assessments and evaluations of patients' mental health status, utilizing the Diagnostic and Statistical Manual of Mental Disorders (DSM) to inform diagnoses and treatment planning.

• Develop individualized care plans that address the unique needs of each patient, incorporating cultural diversity and special considerations relevant to the demographics of the population served.

• Monitor and evaluate patient progress through ongoing assessments, modifying treatment strategies to enhance the effectiveness of care provided.

• Document all treatment records and clinical information accurately and in a timely manner, adhering to regulatory requirements while utilizing current healthcare technologies.

• Collaborate effectively with patients, their families, and other healthcare professionals to construct comprehensive treatment plans that consider desired outcomes and the overall mental wellness of the patient.

• Facilitate referrals to external resources and maintain positive relationships with community providers, ensuring a seamless continuum of care for patients.

• Provide education and mentorship to interns, residents, and volunteers, fostering an environment of knowledge sharing and clinical skill development.

• Contribute to the professional development of team members through peer reviews, clinical collaborations, and chart audits, under the guidance of clinical supervision.

Patient Population / Client Focus\n

Your focus will be on a diverse client population, consisting of individuals facing various mental health challenges. This role requires a deep understanding of cultural sensitivities and the ability to tailor treatment protocols to meet the distinct needs of each patient, including underserved communities in Anaheim and its surrounding areas.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution is required.

• Active LCSW license in the State of California, demonstrating eligibility to practice independently.

• Proficiency in electronic health record (EHR) systems and other relevant technological platforms.

• Physical ability to perform job duties including standing, walking, bending, and lifting up to 25 pounds.

• Commitment to continue professional development and training as required by state regulations.

Preferred Experience\n

• A minimum of two years of direct clinical experience working as a licensed clinical social worker in a mental health setting.

Compensation and Benefits\n
