This role is performed in a remote setting, providing flexibility while maintaining close collaboration with a dynamic team of mental health professionals, including psychologists, occupational therapists, and medical staff. Although the work is conducted remotely, it is essential for team members to engage regularly in virtual meetings and discussions for case reviews and collaborative care planning. The position also allows for face-to-face interactions with clients, ensuring personalized support tailored to their needs.

Key Responsibilities\n

• Conduct comprehensive assessments that involve discussions with clients regarding their personal goals, preferences, and available options for treatment.

• Develop individualized care plans that reflect each client’s education levels, personal strengths, and aspirations for future employment, continuously modifying these plans based on client progress.

• Collaborate with a diverse team of healthcare professionals to implement holistic care strategies that best fit the specific needs of each client.

• Maintain accurate and up-to-date case files and records for clients, including documenting detailed client histories, eligibility for services, insurance information, and correspondence.

• Monitor and evaluate client progress consistently, using established metrics to determine if treatment goals are being met and adjusting plans as necessary.

• Provide ongoing support to clients as they engage in job training, occupational programs, or other supportive interventions.
\n
• Liaise with clients’ families and support systems to create a collaborative support network that facilitates effective treatment.

• Compile and analyze client needs and document reports systematically to ensure continuity of care.

• Make informed recommendations and referrals to appropriate community resources and support services.

Qualifications\n

• A Master's degree in a relevant discipline such as counseling, social work, psychology, or marriage and family therapy is required.

• Possession of a current Licensed Mental Health Counselor (LMHC) credential is essential to practice.

Compensation and Benefits\n
