Work Environment

This role is primarily onsite within a home care environment, enabling you to provide direct and personalized support to patients in their familiar settings. You will work collaboratively in a multidisciplinary team of healthcare professionals dedicated to pain management and patient-centered care. The work schedule generally spans from 9:00 a.m. to 6:00 p.m., with flexibility for occasional evening clinics and participation in community outreach events during weekends, culminating in an average of 40 hours per week.

Key Responsibilities

• Perform thorough health assessments and screenings for patients dealing with pain management issues, allowing you to develop tailored care plans that cater to their specific health needs.
• Diagnose and manage both acute and chronic conditions, focusing on pain relief strategies, preventive care, and promoting overall health and wellness.
• Engage in collaborative efforts with local community organizations to create health education initiatives, workshops, and resources that raise awareness about pain management and health equity.
• Implement culturally sensitive practices, advocating for patients' rights and ensuring they have access to the necessary healthcare services they require.
• Coordinate referrals and ensure follow-up care, fostering continuity in patient care that aligns with best practices in pain management.
• Document all patient interactions meticulously, maintaining accurate medical records that comply with healthcare regulations and organizational standards.
• Participate actively in community outreach initiatives and health fairs to educate the public about pain management and available resources.
• Stay abreast of community health trends and evidence-based practices to enhance service delivery and patient outcomes.

Patient Population / Client Focus

You will primarily serve diverse populations in the Trenton area, which can include individuals facing chronic pain, acute painful conditions, and those requiring preventive care. The position emphasizes a commitment to serving underserved and vulnerable communities, ensuring that every patient receives compassionate and comprehensive care tailored to their cultural and socio-economic backgrounds.

Qualifications

• A current and valid certification as a Nurse Practitioner.
• An active nursing license in the state of New Jersey.
• A Master’s Degree in Nursing from an accredited institution.
• At least three years of clinical experience, ideally within community health or public health settings, specializing in pain management or similar fields.
• Exceptional interpersonal and communication skills, essential for working effectively with patients from diverse backgrounds.
• A strong commitment to social justice and health equity, with a passion for advocating for vulnerable populations.

Preferred Experience

• Experience working in home care or community health settings, particularly in pain management.

Compensation and Benefits
