Position Overview

Horizon Mental Care Services is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team. This full-time, remote role will focus on providing exceptional care for patients within our hospice center in Pasadena, California. Key Responsibilities

• Conduct comprehensive clinical assessments to evaluate patient needs, psychosocial status, and readiness for treatment.
• Develop and implement individualized treatment plans, incorporating evidence-based practices to address specific challenges faced by patients and their families.
• Facilitate regular therapy sessions, including both individual and group interventions, focusing on psychoeducation and process-oriented discussions to empower patients.
• Coordinate discharge appointments and ensure patients and their loved ones are prepared and informed prior to transition.
• Communicate effectively with referral sources and outpatient providers during admissions, throughout treatment, and at discharge to ensure seamless continuity of care.
• Support patients through crisis situations by employing de-escalation techniques and maintaining a safe therapeutic milieu.
• Participate actively in clinical outreach initiatives, education tours, and professional development opportunities to foster community engagement.

Patient Population / Client Focus

In this role, you will primarily work with patients in hospice care who are experiencing complex emotional and psychological issues related to terminal illness. This includes providing support to patients and their families, helping them navigate end-of-life decisions, and facilitating discussions to enhance understanding and acceptance of the dying process. Your empathetic approach will be crucial in addressing their grief, anxiety, and existential concerns.

Qualifications

• A Master’s Degree in Social Work from an accredited institution is required.
• Possession of an active California state license as a Clinical Social Worker (LCSW).
• Demonstrated expertise in therapeutic techniques and a solid understanding of mental health disorders, with a focus on end-of-life care.
• Strong written and verbal communication skills, allowing for effective interactions with patients, families, and healthcare professionals.
• Proficient in timely and accurate documentation of services provided, ensuring compliance with healthcare regulations and the organization’s standards.

Preferred Experience

• Prior experience in hospice or palliative care settings is strongly preferred.
• Familiarity with electronic health records (EHR) systems to manage patient documentation and treatment plans efficiently.
• Proven ability to establish rapport and maintain professional boundaries with clients from diverse backgrounds.

Compensation and Benefits
