Key Responsibilities\n

• Independently manage a designated caseload of students, conducting comprehensive assessments to evaluate their social, emotional, financial, and cultural backgrounds that may impact their school experience.

• Utilize an array of therapeutic interventions to address and mitigate psychosocial barriers, empowering students and their families to actively participate in their treatment plans.

• Serve as a crucial member of the healthcare and educational teams by coordinating care plans, leading discussions on advance directives, and managing crisis intervention services effectively.

• Maintain open communication with case managers and other team members to ensure a cohesive approach to education and psychosocial services tailored to each student's needs.

• Supervise and mentor a team of social workers, interns, and volunteers, fostering their professional development while maintaining high standards of service delivery.

• Provide education and resources to families and caretakers, ensuring they fully understand available supports and how to access them to prevent unnecessary hospital readmissions.

• Respond promptly to psychosocial service requests, ensuring that all interventions comply with discharge plans and timelines.

• Participate in multidisciplinary team rounds, patient care conferences, and training sessions to continuously enhance team collaboration and efficacy.

• Plan and execute care coordination activities specifically for children, liaising with community organizations that will monitor and support ongoing clinical needs.

Qualifications\n

Compensation and Benefits\n

The successful candidate will receive an annual salary in the range of USD 120,000 to 130,000, reflective of experience and qualifications.
