Key Responsibilities\n

• Act as a liaison between clients, their families, and outpatient providers to ensure effective communication and continuity of care.

• Engage with insurance companies to facilitate pre-certification and manage utilization effectively, ensuring clients have access to necessary services.

• Participate in discharge planning and aftercare processes, collaborating with clients and their support networks to develop comprehensive transition plans.

• Provide therapeutic meal support, helping clients to foster a healthy relationship with food through mindful practices and positive reinforcement.

• Deliver personalized, empathetic care, respecting each client’s unique context and intersecting identities, while addressing their specific needs and goals.

In this role, you will work predominantly with individuals struggling with addiction, mental health disorders, and related challenges. Compensation and Benefits\n

The compensation for this role ranges from USD 90,000 to 105,000 annually, reflecting the importance and responsibilities of the position.
