Community Health Coach: Empower Healthy Living & Coaching

Methodist Dallas Medical Center

Methodist Health System in Dallas, TX, is seeking a Community Health Worker to collaborate with a multidisciplinary team to coach patients and link resources by telephone or home visits, aiming to improve healthy living and behavioral change.
