As a telehealth-based position, this role allows counselors to conduct sessions remotely, utilizing safe, HIPAA-compliant platforms to interact with clients. Key Responsibilities\n

• Collaborate with primary care providers to identify, address, and manage mental health and psychosocial issues, ensuring a seamless integration into the primary care team.

• Conduct thorough patient intakes, focusing on diagnostic evaluations to advise primary care providers on treatment goals tailored to individual patient needs.

• Provide short-term therapeutic interventions for clients requiring immediate mental health support, developing personalized treatment plans to foster improvement.
\n
• Maintain ongoing communication with primary care providers, advising them on patient referrals to specialty mental health facilities when necessary.

• Engage in follow-up care, monitoring patient progress and adapting treatment strategies as required to enhance therapeutic outcomes.
\n
• Create and implement relapse prevention plans, assisting clients in achieving and maintaining stable functioning.
\n
• Identify at-risk patients and develop proactive plans to avert worsening mental health conditions through timely intervention and support.
\n
• Oversee the coordination of all behavioral health services received by patients, establishing connections with external treatment providers as needed.
\n
• Document patient progress accurately in treatment charts, ensuring compliance with clinical standards and providing comprehensive updates to healthcare teams.

• Deliver educational resources and self-management skills to empower clients in their treatment journeys, fostering independence and informed decision-making.

• Consult with clinic management on behavioral health services, contributing to program evaluation and enhancement initiatives.

• Ensure adherence to the Code of Ethical Conduct for Clinical Professionals while performing all duties.

• Be willing to take on additional responsibilities as needed to support the practice and its clients.

Qualifications\n

• A Master’s Degree in Clinical Counseling or a related field is required.

Compensation and Benefits\n
