As a key member of our remote team, you will provide therapeutic support to clients, enabling their emotional, mental, and physical growth through evidence-based interventions and positive role modeling.

This position operates within a remote work setting, which allows for flexibility while maintaining a strong connection with our clients and interdisciplinary team. As a member of our Community Health Clinic, you will collaborate with primary care providers and other health professionals to ensure comprehensive care for our patients, utilizing digital tools and secure communication platforms to facilitate consultations and case discussions.

Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, contributing as an integral member of the primary care team.

• Conduct thorough patient intakes focused on diagnostic evaluations and functional assessments, providing insightful recommendations for treatment planning.
\n
• Deliver brief, targeted interventions for clients experiencing a variety of mental health challenges.
\n
• Advise primary care providers on patient management decisions, identifying those who may benefit from specialty mental health services.
\n
• Initiate follow-up with clients to monitor progress, assess treatment efficacy, and adjust care plans as necessary.
\n
• Design and implement relapse prevention strategies to support patients in maintaining stability.
\n
• Detect at-risk patients, formulating prevention plans to avert regression in their condition.
\n
• Oversee and coordinate behavioral health services, interlinking with other providers when authorized by patients.
\n
• Maintain accurate documentation of patient progress and diagnostic data in compliance with agency standards.
\n
• Regularly inform primary care providers about patient needs and advancements to collaboratively adjust treatment plans.
\n
• Empower patients by providing them with self-management skills and information to foster their engagement in recovery.
\n
• Consult with clinic management and team members on behavioral health services, contributing suggestions for program evaluation and outcomes enhancement.
\n
• Support patients in adhering to any prescribed medical treatments, helping them navigate side effects and coping strategies.
\n
• Uphold the Code of Ethical Conduct for Clinical Professionals and participate in other duties as assigned.

Patient Population / Client Focus\n

Your role as an LPC will center around diverse patient populations that include adolescents, adults, and families facing various mental health challenges. You will interact with clients from different backgrounds, tailoring therapeutic approaches to meet their unique needs while fostering a safe, inclusive environment for healing and growth.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a related field.

• Current LPC (Licensed Professional Counselor) certification is mandatory.

• Comprehensive knowledge of mental health assessment techniques, treatment planning methodologies, and psychosexual therapy methods.

• Understanding of developmental psychopathology and psychosocial systems related to family dynamics, including knowledge of DSM-IV criteria.

• Ability to communicate effectively in both English and Spanish is preferred, enhancing our outreach in the community.

• Adherence to the highest standards of mental and physical health suitable for the role.

• Profound understanding of how mental health affects individuals and families, and familiarity with community resources.

• Evidence of strong skills in psychodiagnostics and psychotherapeutic practices.

• A demonstrated capacity to evaluate the individual needs of patients.

Preferred Experience\n

• Experience working in a community health or multidisciplinary care setting is highly desired.

Compensation and Benefits\n

This role offers a competitive salary range of USD 87,000 to 105,000 per year, depending on experience and qualifications.
