Key Responsibilities\n

• Conduct thorough assessments to identify the mental health needs of clients, tailoring your approach to ensure effective care.

• Communicate effectively with interdisciplinary teams, including supervisors, direct care staff, families, and guardians, to discuss patient progress and treatment plans.

• Create and maintain a tentative schedule for client appointments, home visits, and observational activities to monitor treatment effectiveness.
\n
• Coordinate home visits to conduct in-home observations, review charts, and gather relevant data to inform care strategies.
\n
• Oversee the implementation of treatment plans, ensuring they align with best practices and meet client needs.
\n
• Participate actively in interdisciplinary team meetings, contributing insights and updates on client progress and care coordination.
\n
• Document monthly and quarterly progress notes accurately to reflect clients’ evolving needs and treatment outcomes.
\n
• Organize and facilitate training sessions for staff to ensure implementation competencies regarding treatment plans and documentation requirements.

• Provide follow-up support through ongoing interviews, home visits, and observational assessments for continuous care.
\n
• Maintain organized records to facilitate information-sharing among various stakeholders, promoting transparency and collaboration.

• Engage with the broader mental health community to support development goals that enhance client care and community awareness.

Patient Population / Client Focus\n

In this role, you will work with a diverse client population, including both adults and youth. You will address a variety of mental health concerns, including anxiety, depression, trauma, and substance misuse. Your skills will be instrumental in fostering a supportive environment where clients feel safe to express their challenges and work towards their therapeutic goals.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of New Jersey.

• A minimum of two years of counseling experience, preferably in a clinical setting, demonstrating the ability to manage diverse client cases.

• Strong knowledge of legal and ethical standards governing mental health practice.
\n
• Exceptional interpersonal and communication skills, with a proven ability to build rapport with clients from various backgrounds.

• A track record of effective collaboration in a multi-disciplinary healthcare team.

• Familiarity with electronic medical record (EMR) software and related technologies to enhance practice efficiency.
