Work Environment

As a Physical Therapist at Down East Care, your work setting will primarily be in patients' homes. Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians to assess their specific needs and goals.
• Develop and implement individualized treatment plans that address the unique physical therapy needs of each patient.
• Utilize evidence-based therapeutic techniques and methodologies, including manual therapy, exercise prescription, and functional training to achieve optimal patient outcomes.
• Deliver and monitor skilled therapeutic services in patients' homes, adapting treatment techniques as necessary to ensure alignment with the physician's plan of care.
• Provide education and training to patients and caregivers on treatment protocols and preventative care techniques to foster independence and enhance functional capabilities.
• Document progress and outcomes accurately, adhering to regulatory compliance and organizational standards.
• Collaborate with interdisciplinary teams, including physicians and other healthcare professionals, to enhance treatment effectiveness and optimize patient care.
• Meet and exceed performance goals related to patient utilization, satisfaction, and recovery outcomes.

Patient Population / Client Focus

Your role will encompass treating a diverse patient population, specifically focusing on individuals with orthopedic, pediatric, neurologic, and sports-related conditions. You will be instrumental in helping patients navigate the challenges of recovery post-injury, surgery, or chronic conditions, ensuring they have the support and resources necessary to achieve their rehabilitation goals.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program.
• A valid state license to practice physical therapy.
• Basic Life Support (BLS) certification from the American Heart Association.
• Excellent verbal and written communication skills, with a strong ability to build rapport with patients and their families.
• Proficient in computer usage, documentation software, and word processing skills.

Preferred Experience

• A Doctorate of Physical Therapy (DPT) from an accredited institution is strongly preferred.
• Two years of clinical experience in diverse settings with a strong focus on patient care and rehabilitation.
• Current CPR certification is highly desirable.

Compensation and Benefits
