Occupational Therapist

Evaluate residents’ functional abilities, including activities of daily living (ADLs), mobility, cognition, and psychosocial factors.

Develop and implement individualized occupational therapy treatment plans based on evaluations and physician orders.

Provide direct occupational therapy services in compliance with state, federal, and facility regulations.

Monitor and document resident progress, making adjustments to treatment plans as needed.

Participate in interdisciplinary care planning meetings and collaborate with the care team.

Educate residents, families, and caregivers on strategies to enhance safety and independence.

Recommend and instruct on adaptive equipment and assistive devices.

Supervise and provide direction to Certified Occupational Therapy Assistants (COTAs) and therapy aides.

Ensure accurate, timely, and regulatory-compliant documentation in electronic medical records.

Maintain required licensure and complete continuing education per state regulations.

Support quality improvement initiatives and comply with CMS and facility guidelines.

Work primarily in a Skilled Nursing Facility (SNF)/Long-Term Care setting, which may require standing, lifting, and patient transfers.

Work effectively in a collaborative, fast-paced interdisciplinary environment.
