Work Environment

The Physical Therapist position is onsite, delivering services directly within patients' homes. This setting allows you to build meaningful relationships with patients in a comfortable environment while applying tailored therapeutic interventions. Key Responsibilities

• Comply with Virginia state practice acts and relevant regulations governing physical therapy services.
• Conduct comprehensive examinations and evaluations of patients with functional limitations and disabilities to develop effective and individualized treatment plans.
• Utilize evidence-based practices to implement treatment protocols based on thorough evaluation findings and patient presentations.
• Document evaluation results, outline the plan of care, and establish measurable goals to guide patient progress.
• Monitor patient progress and make necessary modifications to treatment plans based on ongoing assessments and changing conditions.
• Communicate and collaborate effectively with other healthcare professionals to ensure integrated care for patients.
• Conduct discharge planning and follow-up evaluations, ensuring patients are equipped with the tools and resources needed for continued health improvement.
• Participate in team meetings and training sessions to enhance service delivery and foster a culture of continuous improvement.
• Maintain meticulous and timely documentation of patient care activities, capturing the patient's journey and ensuring compliance with state and federal regulations.

Patient Population / Client Focus

This role will involve working with a diverse patient demographic, primarily focusing on individuals requiring rehabilitation for orthopedic conditions, post-operative care, and chronic pain management. You will employ a compassionate approach to understand each patient's unique journey, helping them overcome challenges and achieve their wellness goals.

Qualifications

• A Bachelor's Degree in Physical Therapy is required; a Doctorate of Physical Therapy (DPT) is preferred.
• A valid license to practice as a Physical Therapist in Virginia.
• A minimum of 2 years of outpatient orthopedic experience.
• Current CPR certification is necessary.

Preferred Experience

• Experience in home care settings is advantageous, allowing for an understanding of the unique challenges faced in delivering care outside of a traditional clinic environment.
• Familiarity with electronic health records (EHR) systems to capture patient care documentation accurately and efficiently.

Compensation and Benefits
