Key Responsibilities

• Conduct thorough physical therapy evaluations and reassessments based on physician referrals and clinical judgment.
• Develop and implement individualized treatment plans tailored to each resident's specific rehabilitation goals.
• Utilize evidence-based therapeutic interventions to manage pain, enhance strength, and improve balance and gait.
• Maintain accurate and timely documentation within our Electronic Medical Record (EMR) systems, including treatment plans, daily progress notes, and discharge summaries.
• Educate and guide residents, their families, and facility staff on safety, mobility strategies, and home exercise programs to promote proactive recovery.
• Collaborate with a diverse multidisciplinary team to ensure holistic patient care and effective treatment outcomes.
• Adhere to all applicable state and federal regulations, as well as facility policies to maintain compliance and uphold safety standards.

Patient Population / Client Focus

The Physical Therapist will work predominantly with geriatric populations residing in our skilled nursing facility. This role demands an understanding of the complexities involved in rehabilitating individuals with various medical conditions, including those recovering from surgeries, strokes, and chronic illnesses. You will engage with 1-2 patients per hour, allowing for focused and individualized attention, thus enhancing their recovery journey.

Qualifications

• A valid and active Physical Therapist license in the state of Virginia.
• Graduation from an accredited Physical Therapy program (DPT, MSPT, or equivalent).
• Current CPR/BLS certification or a willingness to obtain it upon hire.
• Strong interpersonal skills and clinical reasoning abilities to effectively communicate with patients and their families.
• A sound understanding of physical therapy practices, specifically for geriatric patients and those with complex medical needs.

Preferred Experience

• A minimum of 2 years of experience as a Physical Therapist, preferably in a Skilled Nursing Facility or related post-acute care setting.
• Familiarity with EMR systems, such as Rehab Optima, NetHealth, or PointClickCare, to streamline documentation and patient management.
• Experience with Medicare documentation practices and understanding of PDPM requirements.
• Knowledge of best practices related to fall prevention, wound care, mobility protocols, and post-surgical rehabilitation.

Compensation and Benefits

We offer a competitive salary range of $90,000 to $100,000 per year, commensurate with experience, alongside a comprehensive benefits package that includes health insurance, dental coverage, retirement plans, and paid time off.
