Work Environment

The Physical Therapist position is primarily onsite, providing home care services directly to patients. Key Responsibilities

• Conduct thorough screenings, evaluations, and diagnostic assessments of patients' physical conditions, documenting findings meticulously to develop personalized treatment plans.
• Utilize a range of physical therapy modalities and procedures to treat injuries, diseases, and disabilities according to best practices and patient-specific goals.
• Educate and train patients and their caregivers on effective treatment methods, ensuring they are equipped to continue recovery outside of therapy sessions.
• Effectively maintain clinical documentation and records for each patient, complying with regulatory standards and ensuring timely submission for billing purposes.
• Oversee and collaborate with physical therapy assistants, aides, and support staff, ensuring consistent delivery of high-quality patient care.
• Engage in ongoing professional development through continuing education opportunities to enhance clinical skills and knowledge.
• Foster communication and build rapport with patients, families, and the interdisciplinary team, advocating for the patient's needs and treatment goals.
• Implement and monitor quality improvement initiatives to enhance clinical outcomes and patient satisfaction continuously.

Patient Population / Client Focus

This position will involve working with a diverse patient population across all age groups, including neonates, children, adolescents, adults, and geriatric clients. You will be instrumental in developing tailored treatment plans aimed at improving mobility, reducing pain, and facilitating recovery from various conditions, including post-surgical rehabilitation and chronic illnesses. Your ability to adapt treatment modalities to serve diverse cultural perspectives will enhance patient engagement and results.

Qualifications

• A current state Physical Therapist license.
• A Bachelor's degree in Physical Therapy; a Master's or Doctor of Physical Therapy (DPT) degree from an accredited institution is preferred.
• Minimum of two years of relevant clinical experience is required, with a strong background in a healthcare setting.
• Proficient with various physical therapy techniques and modalities appropriate for the patient demographic served.
• Exceptional verbal and written communication skills are essential for effective patient interaction and documentation.

Preferred Experience

• Experience in home health physical therapy, with a demonstrated ability to work independently and manage diverse patient needs.
• Proficiency in electronic medical record (EMR) systems and familiarity with billing processes will be beneficial.
• Strong team player with experience in a collaborative setting focusing on quality patient care.

Compensation and Benefits
