Key Responsibilities\n

• Conduct thorough assessments of patients to evaluate their mental health needs and create tailored treatment plans.

• Maintain consistent communication with supervisors, healthcare staff, and family members to discuss patient progress and adjust care plans as necessary.

• Organize and provide a detailed schedule for patient appointments and home visits, ensuring effective monitoring of treatment plans.

• Coordinate home visits and in-person community engagement to facilitate patient support, including chart reviews and data analysis.

• Actively participate in interdisciplinary team meetings to share insights and collaborate on holistic care strategies.

• Maintain comprehensive documentation of patient interactions, progress notes, and treatment updates in compliance with legal and ethical standards.

• Organize and deliver training sessions for team members on implementing care strategies and documentation best practices.

Qualifications\n

• Current, active licensure as a Licensed Professional Counselor (LPC) in Colorado.
