Key Responsibilities\n

In this role, you will be responsible for a range of duties that contribute directly to the well-being of our clients:

• Conduct thorough assessments and develop tailored treatment plans, including pre-crisis and crisis plans, behavioral guidelines, risk assessments, and medication treatment plans.

• Maintain compliance with required documentation standards, ensuring that all records meet regulatory guidelines and are completed in a timely manner.

• Utilize evidence-based screening tools and methodologies to monitor client progress and adjust treatment plans as needed.

• Collaborate with multidisciplinary teams to design and implement effective positive behavior support plans that reflect individual client needs.

• Actively engage in licensing and accreditation processes, enhancing the quality of care provided within the organization.

• Serve as a liaison between clients and relevant service providers, including state agencies and community partners, to ensure a unified support system.

• Foster open lines of communication among team members to uphold contract standards and promote Metro West's mission within the community.

• Establish and nurture relationships with local, state, and federal organizations to promote our programs and seek partnerships that align with our goals.

Patient Population / Client Focus\n
