On-Site Case Manager: Housing Placement & Support

SPECIAL SERVICE FOR GROUPS, INC is seeking an On-Site Case Manager to support Pathway Home participants, screening and managing cases on-site and coordinating housing transitions under supervision. You will work with DHS, LAHSA, and partner agencies, enter data in CLARITY/HMIS, and prepare reports. An associate degree in social services or related field is required, with strong communication and Word/Excel skills. #J-18808-Ljbffr Special Service for Groups, Inc.
