Overview
Registered Nurse – Hospice travel assignment in Sonora, California. Start date 09/21/2026 for a 13-week contract in a short-term acute care hospital setting. Weekly pay range is $2,583–$2,823 with 36 guaranteed hours per week. Shift: 8:00 AM–4:30 PM. The role entails end-of-life care delivered within hospice philosophy and regulatory guidelines, supported by an interdisciplinary team.

Responsibilities
- Conduct initial and ongoing assessments of patients’ physical, emotional, psychosocial, and spiritual needs; develop and update individualized care plans with the physician and interdisciplinary hospice team.
- Deliver skilled nursing care, including medication management, pain and symptom control, and wound care; educate patients and families on disease processes, symptom expectations, medications, and hospice philosophy.
- Provide emotional support and crisis intervention to patients and families as needed.
- Serve as the primary case manager for an assigned caseload (typically 12–18 patients), ensuring continuity of care.
- Communicate regularly with physicians, social workers, chaplains, hospice aides, and other team members; participate in interdisciplinary team meetings to review and revise care plans.
- Ensure timely and accurate documentation in the electronic medical record (EMR) in compliance with regulatory requirements; maintain compliance with Medicare, Medicaid, and state/federal hospice regulations. Participate in the on-call rotation and respond promptly to after-hours patient needs.

Requirements
- RN license in the state of practice (California) or as required per assignment
- CPR or BLS certification from an approved vendor
- Education: BSN preferred; Bachelor’s Degree in Nursing is preferred
- Experience: Minimum two years of recent nursing experience; hospice experience preferred; five years hospice nursing experience preferred
- Certifications: Current BLS/CPR certification; RN licensure required
- Licensure: Valid driver’s license; at least 21 years of age; current automobile insurance and reliable vehicle
- Ability to work in a hospice setting and manage a caseload of 12–18 patients; strong communication and teamwork skills
