Key Responsibilities\n

• Conduct comprehensive assessments of clients to determine their social, emotional, and environmental needs, facilitating personalized strategies for intervention.

• Craft detailed service plans that delineate specific goals, interventions, and methods for tracking progress, ensuring alignment with clients’ continuous evolving needs.

• Provide individual and family counseling utilizing evidence-based practices to address complex life challenges such as grief, trauma, addiction, and transactional relationships.

• Engage in active collaboration with clients to create tailored service plans that outline pathways to resources and community support tailored to their unique situations.

• Deliver crisis intervention services, including risk assessments and safety plans, preparing for and addressing emergencies or escalating crises adeptly.

• Coordinate with community agencies and organizations to streamline access to services for clients, functioning as an advocate to enhance client care and support.

• Participate in community outreach initiatives to raise awareness of available services, fostering community engagement and building relationships with potential service users.

• Attend case conferences, staff meetings, and training sessions focused on professional development, teamwork, and adherence to best practices.

Compensation and Benefits\n
