Key Responsibilities\n

• Conduct thorough initial assessments and evaluations of clients to determine psychosocial needs, strengths, and areas of concern.

• Develop, implement, and adjust individualized treatment plans that include short- and long-term therapeutic goals.

• Facilitate both individual and group therapy sessions, focusing on coping strategies, emotional processing, and stress management.

• Provide psychoeducation to clients and their families, helping them understand the illness and guiding them in navigating available resources.

• Maintain accurate and timely documentation of clinical interactions, treatment progress, and patient notes in accordance with regulations and best practices.

Patient Population / Client Focus\n

The primary focus for our LCSW is on patients receiving hospice care, including individuals with terminal diagnoses and their families. Compensation and Benefits\n
