This position operates within a remote outpatient office setup, allowing flexibility for professionals to manage their schedules while maintaining a strong commitment to patient care. Our facility is open Monday through Saturday, providing ample opportunity to connect with clients at times that suit their needs. We offer in-house credentialing and support for managing insurance participation, making it easier for you to focus on what matters most—providing quality care.

Key Responsibilities\n

As a Mental Health Counselor at Trinity Health Behavioral Healthcare, your primary responsibilities will include:

• Conduct thorough psychosocial assessments for individuals, families, and groups to identify their needs and establish appropriate treatment plans.

• Engage in collaborative conversations during patient rounds, effectively communicating assessment findings, treatment interventions, and contributions to interdisciplinary care plans.

• Provide timely, ethical documentation reflective of care delivered, complying with departmental policies and ensuring seamless communication across the team.

• Deliver case management services, collaborating with hospital and community professionals to ensure comprehensive and suitable care tailored to each patient’s circumstances.

• Implement Cognitive Behavioral Therapy (CBT) and various clinical evidence-based modalities to help patients achieve their personalized goals and improve their mental health.

Patient Population / Client Focus\n

Your work will involve addressing the needs of a diverse patient population, ranging from children and adolescents to adults and families. The outpatient setting allows for a wide variety of cases and opportunities to make impactful interventions. You will work closely with patients facing anxiety, depression, relationship issues, and more, ensuring you provide tailored support to each individual's unique journey.

Qualifications\n

To be considered for this role, candidates must possess the following qualifications:

• A current and valid license as a LMFT, LMHC, or LCSW in the state of New York.

• A minimum of a Bachelor’s degree in a relevant field, with a strong emphasis on mental health practices.

• Commitment to working flexible hours ranging from 10 to 40 sessions per week, depending on client availability and needs.

Preferred Experience\n

Applicants with at least two years of clinical experience are highly encouraged to apply, as this will enable you to more effectively navigate the complexities of patient care in our supportive environment. Experience in conducting CBT and familiarity with evidence-based treatment modalities will be a plus.
