Key Responsibilities\n

• Lead therapeutic sessions, facilitating psychoeducational and process-oriented therapy groups to promote emotional healing and coping skills among patients.

• Conduct thorough clinical evaluations to assess individuals' psychological, emotional, and social needs as part of the admission process.

• Initiate discharge planning on the day of patient admission to ensure a seamless transition, identifying essential resources and support systems.
\n
• Facilitate effective communication with the Utilization Review Department and Business Department regarding authorization and payment processes.
\n
• Coordinate with outpatient providers and referral sources throughout the care continuum to maintain continuity of care and follow-up support.
\n
• Assess patients for suicidality and potential risks to self or others, ensuring immediate interventions are implemented as necessary.
\n
• Support the milieu staff in their daily interactions with patients to enhance the therapeutic environment, fostering trust and engagement.
\n
• Actively participate in clinical outreach initiatives, community awareness tours, and support programs to promote the organization's services.
\n
• Manage crisis situations during therapy sessions and within the milieu, ensuring patient safety and adherence to clinical protocols.

• Document all clinical interactions accurately and in a timely manner, which includes initial assessments, SMART treatment plans, and clinical case notes.

Patient Population / Client Focus\n

The primary client focus will be on individuals receiving hospice care, which requires a sensitive approach to address both the emotional and psychological aspects of end-of-life challenges. You will engage with a diverse clientele, including patients from varied cultural backgrounds, each facing unique circumstances and requiring tailored support and interventions. The objective is to improve their quality of life while supporting grieving families.

Qualifications\n

• Master's degree in Social Work, Counseling, Psychology, or a related mental health discipline from an accredited institution.

• Must possess an active and independent license as a Licensed Clinical Social Worker (LCSW) in the state of California.
