Work Environment

This role is centered around home settings, where therapists engage with patients in a comfortable and familiar environment. Physical Therapists will work closely with an interdisciplinary team, including nurses, care coordinators, and support staff, to deliver cohesive and effective treatment plans tailored to diverse patient needs.

Key Responsibilities

The primary responsibilities of the Physical Therapist include:

• Designing and implementing individualized physical therapy treatment plans aimed at preventing disability, relieving pain, and restoring function based on thorough evaluations.
• Overseeing the work of Physical Therapy Assistants to ensure the adherence to prescribed care plans and methodologies.
• Conducting comprehensive evaluations of clients' physical capabilities through diagnostic assessments of muscle, nerve, joint, and functional abilities.
• Analyzing and adjusting therapy plans based on evaluations of clients' home environments, recommending modifications to promote independence.
• Administering a variety of therapeutic modalities, including manual therapy techniques, therapeutic exercises, and functional activities, to achieve optimal patient outcomes.
• Collaborating in case conferences to discuss patient progress and adjust treatment plans accordingly.
• Maintaining and documenting accurate clinical records, including initial evaluations, progress notes, and discharge summaries.
• Providing education to clients and families regarding physical therapy devices and home exercise regimens.
• Participating in ongoing professional development and training opportunities to remain current in best practices and methodologies in physical therapy.

Patient Population / Client Focus

You will work with a diverse patient population, including clients recovering from surgeries, managing chronic conditions, or seeking preventative care. Emphasis on patient and family engagement is vital, as you will be responsible for not only treatment but also for empowering clients and their families to participate actively in the rehabilitation process.

Qualifications

The ideal candidate will possess the following:

• A Bachelor's Degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapy licensure in the state of Virginia.
• Active CPR certification.
• A valid driver's license and reliable transportation are essential for community-based care.
• Exceptional interpersonal communication skills, enabling effective collaboration with families and interdisciplinary team members.

Preferred Experience

While a minimum of one year of experience in acute care or home care settings is preferred, candidates demonstrating strong interpersonal skills, a positive attitude, and a commitment to clinical excellence will be considered. Experience supervising Physical Therapy Assistants and familiarity with current patient management regulations will be considered an asset.
