Full-Time Licensed Physical Therapist in Home Care

Work Environment

This position operates primarily in a home care setting, where you will provide therapy services directly in patients' homes. This arrangement allows for personalized interactions that best support the patient's recovery in their comfortable environments. You will have the opportunity to work flexible hours, with a total of 40 hours per week, including variable weekends as needed. We maintain a strong emphasis on teamwork, and you will frequently collaborate with interdisciplinary specialists to ensure comprehensive care for our patients.

Key Responsibilities

• Conduct thorough assessments to evaluate patients' physical abilities, limitations, and rehabilitation needs.
• Design and implement individualized treatment plans that include therapeutic exercises, manually-assisted therapy, modalities, and education tailored to each patient's requirements.
• Provide ongoing education to patients and their families regarding treatment protocols and progress, ensuring they understand the rehabilitation process and their roles in it.
• Collaborate closely with healthcare providers to discuss patient goals, progress, and necessary adjustments to treatment plans.
• Document treatment sessions accurately, maintaining detailed records for compliance with healthcare regulations and to facilitate communication within the client's healthcare team.
• Participate in community outreach initiatives to promote health and wellness, educate the public about physical therapy, and recruit potential clients.
• Supervise and support physical therapy aides and assistants, ensuring quality care and adherence to treatment plans.

Patient Population / Client Focus

Your primary focus will be working with diverse patient populations, including elderly individuals recovering from surgeries, athletes rehabilitating from sports injuries, and patients experiencing chronic pain or mobility challenges. Each patient presents unique challenges, requiring you to adjust your approach dynamically while adhering to established treatment protocols.

Qualifications

• A Doctorate in Physical Therapy is required.
• A current state license to practice physical therapy in Virginia is a must.
• Basic Life Support (BLS) certification is required prior to starting.
• Excellent verbal and written communication skills to facilitate effective interaction with patients, families, and a multidisciplinary care team.
• Proficient in word processing and electronic documentation systems.

Preferred Experience

While a fresh graduate is welcomed, experience in a home health or outpatient rehabilitation setting is highly valued, especially if it includes working with orthopedic patients. Demonstrated ability to manage diverse patient caseloads effectively will set you apart.

Compensation and Benefits
