Work Environment

This position is primarily onsite, with care being provided directly in the client's home. You will interact with a variety of environments and conditions that can challenge your clinical skills and adaptability. The typical work schedule is Monday through Friday, accommodating a 40-hour workweek. Key Responsibilities

• Conduct thorough evaluations of clients' functional abilities through diagnostic assessments involving muscles, nerves, and joint capabilities.
• Design, direct, and implement individualized physical therapy treatment plans that promote rehabilitation, relieve pain, and enhance performance.
• Oversee the work of physical therapy assistants to ensure effective adherence to the established care plans.
• Assess and modify the home environment to ensure a safe and conducive space for therapy and rehabilitation.
• Administer therapeutic exercises, traction, and massages while motivating clients to engage actively in their recovery process.
• Evaluate, fit, and adjust prosthetic and orthotic devices, recommending further modifications to the physician as necessary.
• Document all clinical records meticulously, including initial evaluations, progress notes, and discharge summaries, in compliance with OASIS and other regulatory requirements.
• Actively participate in interdisciplinary team meetings to review patient progress and collaborate on comprehensive treatment plans.
• Provide education and instruction to clients and their families regarding the use of therapy devices and optimal engagement in their therapy programs.
• Maintain clear and timely documentation of clinical and payroll data as per company policy.

Patient Population / Client Focus

This position focuses predominantly on orthopedic patients of varying ages and backgrounds, each requiring tailored rehabilitation plans targeting strength, flexibility, and functional mobility. You will be an advocate for clients, guiding them through the complexities of recovery while promoting independence through supportive techniques. Your ability to build rapport within the home setting will be critical in fostering a successful therapeutic relationship.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A current and valid Physical Therapist license in the state of Virginia.
• At least one year of experience in a physical therapy role, with specific expertise in orthopedics preferred.
• Current CPR certification, ensuring readiness for emergency situations.
• Excellent clinical reasoning skills, effective communication capabilities, and a strong commitment to clinical excellence and integrity.
• A valid driver's license and the ability to maintain valid automobile insurance.

Preferred Experience

• Experience working in a home health or hospice setting.
• Capability of using electronic health records (EHR) and documentation software efficiently.
• Familiarity with patient management systems and methodologies specific to home care therapy.

Compensation and Benefits

The salary for this position ranges from USD 50,000 to 80,000 annually, commensurate with experience and qualifications.
