Key Responsibilities\n

• Conduct initial client assessments to determine goals, preferences, and available options, leading to the development of customized rehabilitation strategies.

• Create and modify individualized rehabilitation plans based on client educational backgrounds and aspirations, evaluating progress and making adjustments as necessary.

• Work collaboratively with a team of medical professionals—including physicians, occupational therapists, and psychologists—to deliver comprehensive rehabilitation programs tailored to individual needs.

• Maintain thorough case documentation, including personal client information, insurance data, correspondence, and treatment notes to ensure a clear continuum of care.

• Continuously monitor and document client progress regarding treatment goals, analyzing outcomes to facilitate timely interventions if needed.

• Engage with clients regularly during their involvement in various programs, addressing any challenges that arise with their job training or placement swiftly and effectively.

• Collaborate with family members and other support systems to develop effective strategies that support client needs outside of therapy.

• Evaluate client requirements and document findings promptly, providing necessary referrals and resources to enhance their rehabilitation journey.

Qualifications\n
