Physical Therapist (PT) – Assisted Living Facility | Port Jervis, NY | Full-Time or Part-Time

Stern at Home Therapy is hiring a Full-Time or Part-Time Physical Therapist (PT) to provide one-on-one physical therapy services at an established Assisted Living Facility (ALF) in Port Jervis, NY .

This is an excellent opportunity for a Physical Therapist looking for the consistency of a facility-based caseload without the driving and unpredictability of traditional home health . Patients are treated at one established facility, so you can focus on patient care instead of spending your day traveling between appointments.

What You'll Do:

As a Physical Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population within the assisted living facility while maintaining clinical autonomy over each patient's plan of care.

Responsibilities include:

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one physical therapy treatments.
• Help patients improve strength, balance, mobility, endurance, safety, and functional independence.
• Provide therapeutic exercise, gait training, transfer training, balance activities, and fall-prevention interventions.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Recommend assistive devices and adaptive equipment when appropriate.
• Educate patients and caregivers on exercises, mobility strategies, and safety.
• Complete accurate and timely clinical documentation.
• Communicate with patients, families, physicians, facility staff, and interdisciplinary care team members.
• Coordinate with Stern's administrative team regarding scheduling, patient care, and logistics.

Compensation:

Full-Time – Minimum 16 Patients

Full-Time W-2:

• $45–$50 per visit.
• Established facility-based caseload.

Full-Time 1099:

• $55 per visit.
• Established facility-based caseload.

Part-Time – Minimum 10 Patients

Part-Time W-2:

• $45–$50 per visit.
• Fixed per-visit compensation.

Part-Time 1099:

• $55 per visit.
• Fixed per-visit compensation.

This facility-based position does not use Stern's standard home-health tiered compensation model.

Qualifications:

• Active Physical Therapist license in New York required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, documentation, and time-management skills.
• Ability to work independently while collaborating with facility staff and the interdisciplinary care team.
• Geriatric, assisted living, skilled nursing, outpatient, or home health experience preferred but not required.
