Clinical Social Workers play a key role in helping patients and families navigate these psychological stressors, providing support across all areas of the hospital. As integral members of the healthcare team, our Social Workers strive to reduce health disparities and promote equity in care. This role, Clinical Social Worker II or III (LCSW) provides comprehensive psychosocial support to patients and families within UCSF Benioff Children’s Hospital – San Francisco in Adolescent Medicine, in both inpatient and outpatient settings. The position involves conducting psychosocial assessments, crisis intervention, counseling, brief therapeutic interventions, consultation, education, community linkage, and facilitation of the plan of care across the healthcare continuum. Department Pulmonary Hypertension inpatient and outpatient department. Responsibilities Provide psychosocial assessment, crisis intervention, counseling, and linkage to community referrals for patients, families, and staff. Act as an active member of the interdisciplinary team, contributing to team goal setting and care planning for complex psychosocial cases. Inpatient CSWII/III develops and implements safe discharge plans; outpatient CSWII/III facilitates continued care planning, addressing biopsychosocial needs and adjustment to illness in collaboration with interdisciplinary team members. Assess, intervene with, and report domestic violence, child/elder abuse and other reportable situations in accordance with legal mandates and UCSF policies. Screen for suicidal/homicidal ideation and collaborate with UCSF Psychiatry, Child or Mobile Crisis, and other teams on cases requiring psychiatric holds or complex behavioral health concerns. Consult on challenging custody issues, clarify decision‑making capacity, identify surrogate decision makers, facilitate completion of Advance Health Care Directive and/or POLST, and coordinate probate conservatorship or court orders as appropriate. Utilize the Diagnostic and Statistical Manual (DSM) for assessment and diagnosis. Conduct substance use and dependence assessment and intervention strategies, address mental illness, end‑of‑life/bereavement support, benefits and entitlements, and adjustment to illness. Comply with Department standards for timely, accurate, and complete documentation in the Medical Record and collect required statistical data. Facilitate patient and family groups and coordinate patient and family meetings with the interdisciplinary team. Attend and may facilitate department, hospital, and community in‑services related to the field of practice. Participate in quality improvement activities and program planning for the Department, Hospital, and Service Lines. Under the direction of the Social Work Supervisor, CSW III provides clinical supervision to CSW II staff for LCSW licensure in compliance with the California Board of Behavioral Sciences (BBS). Participate in coverage as needed and assigned. Comply with all UCSF policies relevant to scope of work. Perform other duties as assigned. Minimum Requirements Master’s degree from a CSWE accredited school of Social Work. Current California State License of Clinical Social Work in good standing (CSW III). Experience (internship/career) in a healthcare, mental health, and/or child protection setting working with children and families. Experience in crisis de‑escalation and trauma‑informed care. Knowledge and abilities essential to the successful performance of the duties assigned to the position. Demonstrates resourcefulness, effective written and oral communication, diplomacy, and organizational and analytical skills. Self‑directed, assertive, and creative in problem solving, systems planning, and patient care management in a high‑volume work environment. Ability to work effectively and collaboratively with interdisciplinary teams. Proficiency with text/pager, telephone, and computer skills, including the use of the internet. Flexibility to orient and work at all UCSF Medical Center locations. Preferred Experience working with pediatric/adolescent patients. Proficiency in a second language, preferably Spanish. Licenses / Certifications Current California State License of Clinical Social Work in good standing (CSW III).
