Key Responsibilities\n

• Act as a liaison between families and outpatient providers to ensure cohesive care and communication.

• Engage with insurance companies to manage pre-certification processes and utilization management, ensuring that clients receive the benefits they need.

• Participate actively in discharge plans and aftercare strategies, aiding clients in their transitions to independent living.

• Provide therapeutic meal support, educating clients about developing a healthy relationship with food while addressing their emotional needs.

• Deliver safe, empathetic, and affirming care tailored to each client, reflecting their unique circumstances and intersecting identities.

• Document client sessions and progress using our electronic health record (EHR) system, adhering to compliance and quality care standards.

Patient Population / Client Focus\n

This role predominately involves working with a diverse client population that includes individuals facing anxiety, depression, trauma, and other mental health issues. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Valid LCSW license in the state of Florida.

Compensation and Benefits\n
