Key Responsibilities\n

• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, and provide informed recommendations to primary care providers regarding treatment plans.

• Deliver brief, solution-focused interventions for clients requiring immediate mental health services, effectively contributing to the overall health of the patient.

• Collaborate with primary care teams to identify and manage mental health and psychosocial issues, ensuring prompt referrals to specialty mental health facilities when necessary.

• Initiate follow-up communications to monitor patient progress and adapt treatment strategies based on evolving conditions.

• Create and implement relapse prevention plans, reinforcing stability and progressive recovery for patients.
\n
• Identify at-risk patients and develop targeted prevention plans to avert worsening conditions.
\n
• Coordinate and monitor the delivery of behavioral health services, ensuring seamless integration with other treatment providers, respecting patient confidentiality.
\n
• Maintain accurate documentation of patient progress and diagnostic information within the treatment chart, keeping all team members informed of changes in patient status or needs.

• Support patients in adhering to any medical treatments prescribed by primary care providers, including strategies for managing medication side effects.

• Uphold the highest ethical standards as outlined by the Code of Ethical Conduct for Clinical Professionals and carry out additional duties as assigned.

Qualifications\n

Compensation and Benefits\n
