Key Responsibilities\n

• Prepare and circulate tentative schedules for client appointments, including home visits and observational assessments to monitor progress effectively.

• Coordinate and execute in-home visits to conduct thorough evaluations, gather data, and provide hands-on support.

• Oversee the proper implementation of therapeutic program plans and consistently evaluate their effectiveness in achieving intended outcomes.

• Engage in interdisciplinary team meetings, contributing valuable insights and collaborating on strategies to enhance patient care.

• Create and maintain accurate monthly and quarterly progress reports documenting client developments and adaptations to treatment plans.

• Lead and organize training sessions to ensure staff competency in executing program guidelines and maintaining precise documentation.

• Provide ongoing support through follow-up interviews and home visits, adapting strategies to meet evolving client needs.

• Maintain comprehensive records that ensure client information is accessible to authorized parties while upholding confidentiality.

• Act as a liaison with the broader mental health community to bolster collaborative development goals.

• Assist in the creation and enhancement of training materials aimed at improving program efficacy for clients.
