Work Environment

This position is fully onsite, where you will provide services directly in clients' homes, adapting to their unique environments. The role requires the ability to travel within Alexandria and surrounding areas, bringing flexibility and comfort to our clients as they receive therapy in familiar settings. You will have the opportunity to foster meaningful relationships with clients and their families while applying evidence-based practices to enhance rehabilitation outcomes.

Key Responsibilities

• Conduct thorough evaluations of client mobility, strength, and functional status to develop tailored therapy plans.
• Create and implement individualized treatment regimens aimed at improving mobility, strength, balance, and daily living activities.
• Utilize a variety of therapeutic modalities, including manual therapy techniques, therapeutic exercises, and the use of assistive devices to help clients reach their goals.
• Employ effective pain management strategies and implement fall prevention techniques tailored to the needs of each client.
• Continuously monitor client progress, documenting changes and adjusting therapy plans as necessary to optimize recovery.
• Educate clients and their caregivers on therapeutic exercises and self-management techniques for ongoing success.
• Collaboratively work with a multidisciplinary team, including nurses, social workers, and physicians, to ensure comprehensive care coordination.
• Maintain compliance with healthcare regulations through accurate clinical documentation and reporting.

Patient Population / Client Focus

Your primary focus will be on diverse populations, including elderly clients and individuals recovering from surgery or managing chronic conditions. Understanding the unique challenges faced by seniors, including mobility issues and age-related conditions, is essential. You will employ a patient-centered approach that respects each client's preferences and encourages their active participation in the therapy process.

Qualifications

• Current and active Physical Therapy license in the state of Virginia.
• A Bachelor's Degree in Physical Therapy or higher from an accredited educational institution, ideally holding a Doctor of Physical Therapy (DPT) degree.
• Strong foundational knowledge of physical therapy modalities, particularly for geriatrics and home health care settings.

Preferred Experience

• A minimum of 2 years of experience working in a home care, assisted living, or skilled nursing facility.
• Experience with diverse therapeutic techniques and rehabilitation strategies tailored for geriatric patients.
• Familiarity with electronic health records (EHR) systems for clinical documentation.

Compensation and Benefits
