Work Environment\n

This role is primarily remote, allowing you the flexibility to manage your schedule while still being an integral part of the Oaks Integrated Health Centre family. Although you will predominantly work from home, you will have access to an outpatient office for necessary in-person consultations and team meetings. Key Responsibilities\n

• Conduct comprehensive assessments to identify the needs and mental health goals of each patient, tailoring treatment plans accordingly.
\n
• Facilitate communication between supervisors, direct care staff, family members, and other relevant parties to provide updates on treatment progress and adjustments needed.
\n
• Develop and share a tentative schedule with supervisors to outline appointments, home visits, and observations, ensuring accountability and progress tracking.
\n
• Coordinate and conduct home visits for in-home observations, chart reviews, staff interviews, and data analysis to provide thorough care and support.
\n
• Implement and monitor the effectiveness of the treatment program plans, ensuring compliance and quality service delivery.
\n
• Engage in interdisciplinary team meetings that promote information sharing and collaborative efforts in addressing patient needs and care strategies.
\n
• Prepare detailed monthly and quarterly progress notes to document advancements and areas needing additional focus.
\n
• Organize and conduct training sessions for staff to ensure proper understanding and implementation of treatment plans and documentation requirements.
\n
• Provide ongoing support through follow-up interviews and home visits to ensure that patient needs are met consistently and effectively.
\n
• Maintain comprehensive patient records to ensure information accessibility for relevant parties, enhancing care continuity.
\n
• Act as a liaison with both the public and the mental health community to advocate for and support patient development goals.
\n
• Assist in creating and refining training materials that enhance program effectiveness for our clients.

Patient Population / Client Focus\n

The LPC will work with a diverse client population, including children, adolescents, and adults facing various mental health challenges. Our clients often seek assistance in managing issues such as anxiety, depression, trauma, relationship difficulties, and substance misuse. Your role will involve tailoring therapeutic approaches to meet the unique needs of each patient while promoting a supportive and welcoming environment.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Possession of active and unrestricted licensure as a Professional Counselor (LPC) in the state of Oregon is mandatory.

• A minimum of two years of experience in a counseling capacity, preferably in a clinical environment.
\n
• Strong foundation in legal and ethical standards relevant to mental health practice.
\n
• Demonstrated experience in establishing rapport with a diverse clientele is essential.

• Proven ability to collaborate effectively with a multidisciplinary healthcare team.

• Proficiency in using EMR (Electronic Medical Records) software and related technologies to document patient interactions and manage records.
