Key Responsibilities

As a Physical Therapist in our facility, you will be responsible for:

• Conducting initial evaluations and detailed assessments of patients' physical capabilities and limitations to establish baselines and develop individualized treatment plans.
• Utilizing a variety of therapeutic techniques, including exercise prescription, manual therapy, and the application of modalities to alleviate pain and improve mobility.
• Implementing and modifying treatment plans based on patients' progress, ensuring that therapeutic goals are met efficiently and effectively.
• Documenting treatment sessions meticulously, tracking progress, and reporting findings to other healthcare team members as needed.
• Empowering patients through education on injury prevention, rehabilitation techniques, and effective use of assistive devices for enhanced independence.
• Maintaining current knowledge of industry trends and best practices through continued education, professional workshops, and participation in professional organizations.

Patient Population / Client Focus

Our patient population ranges from children to senior adults, tackling varied diagnoses such as orthopedic injuries, neurological disorders, post-surgical rehabilitation, and chronic pain management. You will have the opportunity to work closely with patients in a home care environment, tailoring therapy to meet their unique needs and fostering long-term wellness through engagement and education.

Qualifications

To be successful in this role, candidates must possess:

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A current and valid State License to practice Physical Therapy.
• A minimum of two years of professional experience in a clinical setting, demonstrating a solid understanding of patient care modalities.
• Strong interpersonal and communication skills, both written and verbal, to effectively interact with patients and team members.
• Proficient word processing and computer skills to maintain thorough documentation and utilize electronic health record systems.

Preferred Experience

While we encourage candidates with two years of experience in physical therapy, recent graduates with a passion for physical therapy and a willingness to learn will be considered.
