Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to determine the emotional, mental, and social needs of senior clients.

• Engage with clients and their families to formulate individualized care plans that address specific goals, preferences, and life transitions.

• Provide counseling sessions to seniors, aiding in emotional support and helping them cope with various life challenges, such as bereavement or relocation to assisted living.

• Advocate for clients' rights, ensuring they have access to essential community resources like healthcare services, support groups, and social activities.

• Work alongside interdisciplinary teams, which may include nurses, occupational therapists, and case managers, to ensure a well-rounded approach to each client’s care.

• Regularly monitor client well-being through virtual or in-person visits, assessing living conditions and addressing safety or health concerns.

• Facilitate family discussions, helping to mediate conflicts and improve communication regarding client care plans.

• Remain compliant with state regulations and continually update documentation on client interactions, assessments, and treatment plans in the EHR system.

Compensation and Benefits\n

We offer a competitive salary ranging from USD 60,000 to 100,000 annually, commensurate with experience and qualifications.
