Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify patients’ emotional, social, and financial needs, utilizing various assessment tools and methodologies.

• Provide individual and group counseling sessions, addressing issues such as adjustment, grief, and coping strategies for patients facing health-related challenges.

• Collaborate with medical and mental health professionals to develop and implement integrated treatment plans that enhance patient care outcomes.

• Coordinate post-discharge services with community organizations to ensure continuity of care, helping patients transition successfully out of the clinic.

• Facilitate discussions with patients and their families about treatment options, resources available, and coping mechanisms to manage their conditions.

• Participate in team meetings, contributing insights and expertise to improve departmental policies and services related to social work.

• Provide crisis intervention as needed, offering immediate support to those in acute distress.

• Document client interactions and maintain accurate records in alignment with federal, state, and clinic policies.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
