The Guthrie Physical Therapy is seeking a dedicated and skilled Physical Therapist to join our home care team in Port Charlotte, Florida. Work Environment

This role is conducted onsite in patients' homes throughout Port Charlotte, Florida, offering a dynamic and adaptable work setting. Key Responsibilities

• Direct and manage the physical therapy treatment process, focusing on preventing disabilities, relieving pain, and enhancing muscle function in accordance with individual care plans.
• Oversee the physical therapy assistants and ensure they are implementing care plans effectively, providing mentorship and guidance as needed.
• Conduct thorough evaluations of clients' levels of function using diagnostic techniques to create effective treatment interventions.
• Assess and recommend modifications to the home environment that promote independence and facilitate rehabilitation.
• Deliver physical therapy services as prescribed by physicians, tailoring approaches to address patients' unique needs.
• Administer and instruct clients in manual therapeutic exercises, as well as non-manual exercises focusing on functional activities.
• Utilize therapeutic modalities, including traction and massage, to enhance recovery efforts.
• Provide expertise in evaluating and fitting orthotic and prosthetic devices, making necessary recommendations to physicians when required.
• Document all patient evaluations, treatment plans, and progress notes with accuracy, ensuring adherence to compliance and quality standards.
• Engage in regular case conferences with the interdisciplinary team to review and adapt care plans based on patient progress.
• Facilitate education for clients and their families on care regimen and therapy devices to promote understanding and independence.
• Complete additional duties and objectives as requested to support overall company goals.

Patient Population / Client Focus

You will work with a diverse array of patients, including those recovering from surgery, traumatic injuries, chronic conditions, and age-related challenges that affect their daily living activities. The role requires sensitivity and adaptability to address varied patient needs, ensuring their comfort and trust during the rehabilitation process. Your contributions will significantly impact their recovery journeys and long-term well-being.

Qualifications

• A Bachelor's degree in Physical Therapy is required; a Doctorate in Physical Therapy (DPT) is strongly preferred.
• An active license to practice as a Physical Therapist in the state of Florida, with a clean record free from sanctions or disciplinary actions.
• Comprehensive knowledge of physical therapy techniques, especially those relevant to manual therapy and exercise protocols.
• Proficiency in electronic medical records (EMR) software, preferably Web PT, for efficient documentation.
• Outstanding interpersonal skills with an ability to motivate and guide patients.

Preferred Experience

• Experience in a home healthcare setting is advantageous, providing you with insights into the specific needs and challenges of patients receiving care at home.
• Demonstrated success in developing and implementing personalized exercise programs based on patient assessments.
• A solid understanding of community resources that may assist patients in their recovery path.

Compensation and Benefits
