This position is primarily remote; however, specialists will coordinate with teams in our Erie, Pennsylvania outpatient office as necessary. Key Responsibilities\n

• Conduct thorough and empathetic mental health assessments to accurately identify patient needs and determine the most appropriate interventions.

• Utilize standardized assessment tools and comprehensive clinical interviews to evaluate conditions such as anxiety, depression, substance abuse, and the impact of social determinants on mental health.

• Provide evidence-based therapeutic options, including individual, group, and family therapy sessions, aimed at addressing complex mental health and behavioral issues effectively.

• Design and implement personalized care plans that include measurable goals and objectives tailored to each patient's unique circumstances.

• Act as a care coordinator for assigned patients, facilitating referrals to community resources and ensuring seamless continuity of care through follow-ups and ongoing support.

• Maintain accurate and timely clinical documentation of assessments, treatment plans, progress notes, and discharge summaries, adhering to regulatory and organizational guidelines, particularly in an electronic health record (EHR) system.

As a Licensed Mental Health Therapist at Nature's Rhythm, you will serve a diverse patient population, ranging from children to older adults. Qualifications\n

• Minimum of 3 years of recent clinical experience in mental health care.

• A Master’s Degree in Counseling, Social Work, Marriage & Family Therapy, or Psychology from an accredited institution.

• A valid and current State of Pennsylvania License as an LCSW, LMFT, or LPC.

• Ability to effectively collaborate within a multidisciplinary team to create an integrated care model.

• Strong communication, organizational, and problem-solving skills, crucial for facilitating effective therapy and treatment plans.

• Familiarity with electronic health records (EHR) and knowledge of documentation best practices is essential.

Preferred Experience\n

Compensation and Benefits\n

Nature's Rhythm offers a competitive salary ranging from USD 90,000 to 100,000 annually, commensurate with experience.
