Work Environment

This role is primarily onsite, involving travel to patients' homes. Key Responsibilities

• Conduct thorough evaluations of patients in their home settings to assess their physical condition and rehabilitation needs.
• Develop and implement customized treatment plans that incorporate various therapeutic modalities, including exercise, manual therapy, and adaptive equipment.
• Monitor and document patient progress, adjusting treatment plans as necessary to optimize recovery outcomes.
• Educate patients and their families about rehabilitation techniques, preventive care, and the use of assistive devices to enhance independence.
• Provide ongoing support and training to caregivers, ensuring they are equipped to assist patients effectively.

Patient Population / Client Focus

Qualifications

• Possession of a Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• Valid state license in Physical Therapy.

Compensation and Benefits
