Work Environment\n

This role is designed to be performed primarily in a remote setting, allowing for a flexible work experience. However, as part of our outpatient office model, you will interact with clients through virtual appointments, providing you the freedom to balance your professional commitments with personal life. Key Responsibilities\n

• Conduct comprehensive assessments and evaluations to develop tailored treatment plans that effectively address patients' unique mental health needs.

• Facilitate open lines of communication between supervisors, direct care staff, family members, and other stakeholders regarding each patient’s progress and treatment plan.
\n
• Maintain an organized schedule for appointments, home visits, and necessary observations to monitor patient advancement and make adjustments as needed.
\n
• Coordinate home visits that align with the standards for in-home assessments, ensuring thorough chart reviews and data analysis.
\n
• Oversee the execution of treatment plans, ensuring they are implemented effectively and yielding positive outcomes.
\n
• Engage regularly in Interdisciplinary Team Meetings and additional essential gatherings to discuss patient care strategies and progress.
\n
• Draft and complete detailed monthly and quarterly progress notes that adequately reflect each patient’s development.
\n
• Provide training sessions for staff, ensuring all team members are knowledgeable in the execution of treatment protocols and documentation standards.
\n
• Conduct follow-up interviews and home visits to continuously support patients’ needs and assess their ongoing progress.
\n
• Maintain organized and accurate records to ensure all relevant information is accessible to authorized parties.
\n
• Serve as a liaison with community resources and the mental health sector to enhance treatment opportunities and support various development goals.
\n
• Contribute to the advancement of training materials and procedures, aimed at elevating the quality of care provided within our programs.

Patient Population / Client Focus\n

This role will involve working with a diverse array of patients, including both adults and youth facing various mental health challenges. You will have the opportunity to specialize in areas such as family counseling, couples therapy, trauma-informed care, and support for those dealing with substance misuse. This broad client focus allows you to utilize a variety of therapeutic approaches tailored to individual circumstances.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.

• An active and unrestricted license as a Professional Counselor (LPC) within the state of Arizona.
\n
• A minimum of 2 years of relevant counseling experience, preferably in a clinical or outpatient environment.

• A strong foundation in legal and ethical standards governing mental health practice.
\n
• Exceptional interpersonal and communication skills, with a proven ability to build rapport with a diverse clientele.
\n
• Experience working collaboratively with multidisciplinary healthcare teams is highly desirable.

• Proficiency in using electronic medical record (EMR) systems and related technologies to document and track patient progress.

Preferred Experience\n

• Specialization in family therapy, couples therapy, trauma resolution, or substance abuse treatment is highly valued.

• Experience in behavioral health settings that emphasize integrated care processes.
